Nurse Case Manager
Rainbow Rehabilitation Centers, Inc., a leader in the field of community-based rehabilitation in post-acute residential and outpatient settings is seeking a full-time Nurse Case Manager.
Responsibilities
The Nurse Case Manager is responsible for coordinating and closely monitoring rehabilitation programs, communicating with internal and external treatment teams, coordinating care plans, ensuring clients and families receive necessary interdisciplinary treatment services, and writing progress reports.
Requirements
- Self motivated
- Leadership ability
- Ability to work creatively and cooperatively with others
- Desire to work in a fast paced, changing and growing organization
- Problem solving skills and ability to work in a team environment
- Current Michigan licensure
- 1-3 years of related work experience
- Excellent written and verbal communication skills
