Responsibilities:

Administer the Home Care Program in order to ensure that the program is delivered in an appropriate, caring and respectful manne.

  • Ensure that care is provided according to all relevant policies, procedures and regulations.
  • Ensure that Home Care Workers are trained to administer the required care, monitor and evaluate all tasks of the Home Care Worker and the program.
  • Monitor supplies and resources.
  • Identify persons requiring home care and client needs.
  • Make recommendations for changes and improvements to the program, as required.
  • Schedule work assignments with regard to Home Care Worker training and experience.

Discuss any concerns with the supervisor.

  • Coordinate Case Management.
  • Ensure that all members needing care are identified.
  • Coordinate appropriate care and equipment, including other community resources, as required.
  • Establish and maintain a current, accurate, confidential client reporting system.
  • Provide information to other health care professionals, as required.
  • Consult with family members and other supports to ensure that care is on-going and that all client needs are identified and met.

Provide individualized home care.

  • Provide Home Care to elders, disabled persons, and others in the community with identified needs.
  • Provide Home Care including general housekeeping, personal hygiene and Meals-on-Wheels.
  • Transport Home Care clients to appointments and activities.
  • Assist with exercises, physiotherapy and other medical plans.
  • Monitor blood pressure and other physical conditions, as required.
  • Follow medical instructions carefully and consistently.
  • Organize time and resources based on the individual needs of clients.
  • Manage any unanticipated events or unstable situations.

Coordinate community resources and supports.

  • Encourage clients and families to be involved in care, if appropriate.
  • Encourage clients and families to take responsibility for care, if appropriate.
  • Liaise with all family, medical and other resources, as required.
  • Advocate on behalf of clients for additional service and resources, as required.
  • Establish and maintain current, accurate, confidential files for each client.
  • Educate clients, families and support systems about the Home Care Program –what can be provided and when to access other resources.

Perform other related duties as required.

Qualifications:

The Home Care Coordinator requires knowledge of:

  • Client assessment
  • Proper home care techniques including house cleaning, feeding and personal hygiene
  • Appropriate Home Care interventions
  • Services and resources in the community
  • Staff and program management and evaluation
  • All relevant legislation, policies and practices
  • First Aid and CPR

Job Summary:A FirstLight HomeCare Personal Care Assistant is an individual who has completed personal care training and is competent to perform assigned functions of personal care to a client in his/her residence.

Qualifications:An applicant for the position of FirstLight HomeCare Personal Care Assistant will need to meet the following basic qualifications:
The applicant must have completed FirstLight HomeCare's orientation, non-medical training program, be trained in patient privacy issues and demonstrate basic knowledge and competency in the field of senior care.
The applicant must have a sympathetic attitude toward the care of the sick and elderly.
The applicant must have the ability to carry out directions, and to read and write.
The applicant must demonstrate maturity, professionalism and the ability to deal effectively with the demands of the job.
The applicant must be state-licensed (where required) and submit to a complete background-check, including a check of criminal and driving records along with finger printing and substance testing where required.
The applicant must be 18 years of age and a citizen of the United States.

Responsibilities:A FirstLight HomeCare Personal Care Assistant can/will:
Assist clients with personal hygiene, including shower, tub or bed baths, oral care, hair and skin care.
Assist patients in the use of toilet facilities, including bed pans.
Assist clients in and out of bed, excluding the use of mechanical lifting equipment unless trained and documented as competent in that service.
Assist clients with walking, including the use of walkers and wheelchairs, when applicable.
Assist clients with self-administration of medications.
Meal preparation and feeding, when required.
Assist with prescribed exercises when the patient and the aide have been instructed by the appropriate health professional.
Record and report changes in the client's physical condition, behavior or appearance to supervisor or Care Coordinator.
Documenting services delivered in accordance with FirstLight HomeCare policies and procedures.

Working Environment:A FirstLight HomeCare Personal Care Assistant works both indoors in the agency office and in the field with clients and referral sources.

Lifting Requirements:Ability to perform the following tasks if necessary:
Ability to participate in physical activity.
Ability to work for extended period of time while standing and being involved in physical activity.
Heavy lifting.
Ability to do extensive bending, lifting and standing on a regular basis.
