We have an immediate opening for a Social Services Coordinator.

Must be able to work various shifts per week.

Requirements

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Within seven day of admission the SSD will complete a social history and initial assessment of the resident as well as maintain progress notes of all significant contacts with the resident and/or family members
  • Inform resident of Resident’s Rights and other policies                
  • Aid residents and families in establishing a code status that reflects the resident’s wishes as well as assisting with the formation of advance directives  The SDD will review the code status and advance directives annually to ensure they reflect the resident’s wishes
  • Establish relations with the resident and family on admission and will attempt to aid them in adjusting to facility.
  • Work on the care plan committee to appropriate section of the MDS, doing the RAPS, and summaries and advising committee of changes necessary in the plan of care.
  • Assist in assessing the psychical needs of the resident as well as monitoring mood changes and problem behaviors.
  • Meet with resident’s quarterly to assess the resident and inquire about problems the resident may be having and attempt to resolve grievances.
  • Handle all DHS request, attend eligibility appointment for residents, assist in completing PAE’s and make sure that all documents needed to obtain Medicaid medical approval for date requested are received in Nashville
  • Work with families to ensure they understand the Medicaid program and the billing process
  • Complete PASSAR’s for new residents and change in mental status reports
  • Assist resident with Resident Council and maintain minutes of these meetings as well as keeping residents updated on the outcomes of their concerns.
  • Assist Activities if needed with large parties and family meetings
  • Work with the volunteer coordinator to provide for specific resident needs
  • Work with DON to coordinate room changes as needed to ensure the highest level of psychosocial well being for the residents
  • Work with the Quality Assurance Committee to complete studies to ensure quality care.  Record the minutes of these meetings
  • Work with the Case Assessment Team to monitor the changing needs of the residents throughout Appalachian Christian Village
  • Assist residents in congregate living with forms, making calls, conducting assessments, making referrals and assisting with advance directives.
  • Complete discharge planning for all residents and assist with referrals to outside agencies for further services
  • Must have good attendance.
  • Must attend in-service trainings and continued education.
  • Performs other duties as assigned

EDUCATION:

Prefer minimum Bachelor’s Degree in Social Work or equivalent. 

PHYSICAL DEMANDS:

  • To be able to lift up to 25 lbs.
  • To be able to reach, bend, kneel and squat.
  • To be able to walk and stand on a repetitive and constant basis.
  • To be able to use step stools.
  • To be able to hear call system, pager signals, walkie-talkies and spoken English language.
  • To be able to wear protective gloves supplied by facility.
  • To be able to be in contact with domestic pets.
