Key Responsibilities:



Primary Care Delivery



  • Delivers cost-effective, quality care to assigned Bundle Payment members in collaboration with their assigned physicians. 

  • Manages both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty provider.

  • Performs comprehensive assessments and documents findings in a concise/comprehensive manner that is compliant with Bundle Payment visits and documentation requirements and  Center for Medicare and Medicaid Services (CMS) regulations

  • Facilitates agreement and implementation of the member's plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians while ensuring compliance.

  • Evaluates the effectiveness, necessity, and efficiency of the plan of care andmakes revisions as needed.

  • Utilizes practice guidelines and protocols established by Bundle Payment.

  • Promotes the development of a collegial team (for coverage, trouble shooting, and brain storming)

  • Fosters and develops a culture of clinical expertise

  • Serves as a role model to internal and external partners

  • May be required to participate in on-call program.

  • May be required to do home visits

Care Coordination



  • Understands the Bundle Payment benefits and associate policies and procedures, and articulates them effectively to providers, members, and key decision-makers.

  • Assesses the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of any skilled benefit events and communicates process for providers in accordance with Bundle Payment expectations.

  • Coordinates care as members transition through different levels of care and care settings per Bundle Payment protocol.

  • Ongoing monitoring of  needs of members and families while facilitating any adjustments to the care plan as situations and conditions change.

  • Orders and interventions are reviewed for appropriateness and response to treatment to identify most effective plan of care in alignment with the member's needs and wishes.

  • Evaluates plan of care for cost effective services that provide agreed upon outcomes for members, families and providers and decreases high costs, poor outcomes and unnecessary hospitalizations.

  • Transition coaching, educating, coordination in collaboration with the team RN, Facility staff, and PCP.

  • Case Management of all Bundle Payment residents for 90 days post discharge from the hospital; including coordination with facility staff, home care agencies, and PCPs.

  • Regular and effective communication with internal and external parties including physicians, members, key decision-makers, nursing facilities, Bundle Payment staff and other provider groups.

  • Actively promotes the Bundle Payment program in assigned facilities by partnering with key stakeholders (ie: internal sales function, provider relations, facility leader) to maintain and develop membership growth.

  • Exhibits original thinking and creativity in the development of new and improved methods and approaches to concerns/issues.

  • Functions independently and responsibly with minimal need for supervision.

  • Demonstrates initiative in achieving individual, team, and organizational goals and objectives.

  • Participates in Bundle Payment quality initiatives.

 


 

Requirements

Qualifications:


 



  • Nurse Practitioner national certification as ANP, FNP or GNP required.

  • Current unrestricted license in the State of New Jersey required.

  • Proficient computer skills including the ability to document medical information with written and electronic medical records.

  • DEA license or ability to obtain.

  • Prior experience as a Nurse Practitioner in adult, family or geriatrics specialty for a minimum of three years preferred.

  • Excellent communication skills and demonstrated ability to foster a culture of clinical excellence and build collaborative relationships.

  • Prior experience in adult teaching environment a plus.

  • Demonstrated leadership abilities.

  • Prior experience demonstrating astute triage skills

  • Valid driver's license, reliable transportation, and current automobile insurance.
