The CarePlus or Post-Acute Skilled Nursing Facility Nurse Practitioner – Physician Assistant is responsible, as part of the patient care team, for the overall patient care management process. The Nurse Practitioner will be assigned to either provide home visits or to work in skilled nursing facilities, and s/he will not be expected to work in both setting as part of the patient care team, for the overall patient care management process. The NP - PA will be assigned to either provide home visits or to work in skilled nursing facilities, and s/he will not be expected to work in both settings. An advocate for the patients, the NP - PA will provide total care for the patients assigned in collaboration with the physician of record and patient care team. The NP - PA will communicate and manage the patients' care across all care providers, family, and caregivers. The NP - PA will ensure that timely, patient-centered, appropriate evidence-based medicine is provided. The NP – PA will also conduct comprehensive one-time assessments in the home or in a skilled facility for assigned members.
 
Responsibilities include:



  • The Nurse Practitioner will be responsible for the total care of the assigned patient population in collaboration with the Patient Care Team (PCT), and/or Network Clinical Director and community based physicians/providers.

  • This position is accountable for identifying and developing innovative actions to meet the needs of CarePlus patients from both the health care and psychosocial / socioeconomic dimensions of care; as well as taking action for provision of services to meet the patient needs.

  • The NP - PA will be responsible as part of the PCT for patient assessments, risk management plan development, maintenance care, acute interventions within scope, procedures such as IV access and wound care (not all inclusive), documentation and patient education.

  • The NP - PA will share in the responsibilities and care of patients located in understaffed and rural/remote areas of the network and adjacent network(s) as determined through team planning.

  • The NP - PA will ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS, Complex Population Management and client requirements.

  • As part of a call schedule, the NP – PA will participate in rotating call

Requirements

Qualifications:



  • Active, unrestricted Nurse Practitioner or Physician Assistant license in the State of Nevada, or ability to obtain prior to employment

  • Nurse Practitioner Certification as an Adult, Family, or Geriatric Nurse Practitioner from either the ANCC or AANP required, or Physician Assistant Certification

  • Willingness to travel by car to patients' homes or to skilled nursing facilities in assigned locations

 

Assets:



  • Must be able to effectively communicate with elderly and chronically ill patients and families.

  • Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial.

  • Comfortable working independently in a home-visit setting.

  • Ability to draw blood in the home for lab procedures

  • Willingness to perform procedures for self and other provider's patients.

  • Basic knowledge of computers and cell phones.

  • Familiarity with ICD-9 and CPT-4 coding.

  • Bi-lingual or multi-lingual a plus.

  • BCLS

  • Excellent organizational skills with the ability to multi-task.
