The primary purpose of the Field Case Manager is the application of clinical knowledge to coordinate enrollee care needs for assigned members who are primarily medically complex and require intensive medical and psychosocial support.  Coordination of enrollee care needs across the continuum begins with early identification of health risk factors, education, and prevention using the six essential activities of case management as per CMSA.  Coordination also assures the provision of appropriate services in acute, home, chronic and alternative care settings, to meet patient needs in the most cost-effective manner available.

 

Responsibilities:



  • Application of clinical knowledge to affect efficient case management for those accessing their Medicare/Medicaid benefits.


  • Responsibilities for Care Coordinators include:
    -Engaging members telephonically to coordinate services, community resources, and treatment needs
    -Delivering a holistic approach to coordinated care based on the enrollee needs
    -Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
    -Creating, reviewing and revising Plan of Care plans and focusing on disease management
    -Functioning as an advocate for members and decreasing unnecessary hospitalizations
    -Collaborating with the member's PCP to deliver and coordinate necessary services
    -Building relationships with members and their families; assisting them with proper health choices
    -Ensuring cases are documented in a timely manner

Requirements

Requirements:
-Bachelor's level degree in Social Work or other related field
-Minimum of 1 year clinical or case management experience
-Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living

Additional Assets Preferred:
-Medicare/Medicaid experience
-A background working in geriatric special needs
-A background in behavioral health
-Experience with electronic charting
-Ability to multitask
-Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
