  • Screens and assesses specific patient populations as appropriate for nutritional needs; communicates findings and recommendations with physician and other members of health care team.
  • Evaluates enteral feeding prescription including formula type and amount, administration and other parameters, based on specific patient needs, and communicates any need for changes verbally and in writing to ordering physician.
  • May also work with infusion team to assess and monitor total parenteral nutrition (TPN) patients, evaluating formula, monitoring patient status, including labs, and communicating status changes and recommendations to physician and other members of the healthcare team.
  • Contributes to initial and ongoing infusion pathway and care conferences for TPN patients.
  • May set up/demonstrate equipment in the home or hospital, but does not provide hands on patient care.
  • Identifies learning needs of patient and/or caregiver.
  • Provides appropriate instruction for patient/care giver to safely administer enteral nutrition and care at home.
  • Communicates patient learning needs and findings to health care team members, including other clinicians, customer service staff, and physician/referral source, as appropriate.
  • On an on-going basis, monitors patient progress toward collaborative home nutrition goals to assess medical, psychosocial, and home care environment.
  • Ensures patient competence and compliance with all self-care, including procedures and infection control; reeducates as necessary, and documents/communicates the need for reeducation.
  • Maintains current communication with physician regarding patient status and appropriateness of current prescription.
  • Appropriately documents patient care activity, dietitian services provided and all communication with patient and physician.
  • Documentation is accurate, complete, and complies with company policy and procedure for electronic and paper patient clinical Dietitian record entries.
  • Conducts ongoing chart review per policy.
  • Forwards required documentation to physician, payors, and referral sources within policy or contractually stated timelines.
  • Educates staff as appropriate on Medicare pre qualification for home nutrition support.
  • Ensures all necessary information is collected and patient nutrition and caloric needs calculated during intake process, to ensure patient is appropriate for home care.
  • Works with revenue management to ensure prescriptions contain all necessary information for prompt payment.
  • Participates in education and training conferences on home nutrition care.
  • Participates in training sessions of sales personnel marketing the nutrition program. May assist sales personnel in clinical aspects of marketing home nutrition programs.
  • Performs other related duties as directed by supervisor.



Qualifications

  • B.S. degree in Food/Nutrition or Dietetics is required.
  • At least one year related experience is required.
  • A.S.P.E.N. certification is preferred
