East Valley Optum Transitions Nurse Practitioner!


 


The Transitions Nurse Practitioner is responsible, as part of the clinical management team, for the overall medical management process of assigned Transition patients.  An advocate for the patients, the Nurse Practitioner will provide clinical management and oversight for the assigned patients, communicate and manage the patient across all levels of care to include family and caregivers. The Nurse Practitioner will ensure that timely, patient-centered, appropriate evidence-based medicine is provided. 
 
Duties:
The Nurse Practitioner will be responsible for clinical management of the assigned patient population in collaboration with the care management team and/or community based physicians/providers
This position is accountable for identifying and developing innovative actions or plans of care to meet the needs of patients from both the medical care and psychosocial / socioeconomic dimensions as well as taking action for provision of services to meet the patient needs
The Nurse Practitioner will be responsible as part of the Clinical care team  for physical assessments, risk management plan development, maintenance care, acute interventions within scope,  documentation and patient education.
The Nurse Practitioner will ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS, State law, and Health plan requirements
As part of our on call team, the Nurse Practitioner will participate in a call rotation.


Provider will travel the East Valley Corridor.


 

Requirements

Requirements:
Active un-encumbered license to practice nursing in the state where the position is located.
Nurse Practitioner Certification from either the ANCC or AANP.
Geriatric or adult medicine specialty a plus.
Must be able to effectively communicate with elderly and chronically ill patients and families.
Understanding of Medicare, Medicaid and Health Plan benefit structures an asset.
Comfortable working independently, well organized and self-motivated.
Ability to appropriately set and maintain personal and professional boundaries
Basic knowledge of computers and cell phones.
Coding experience beneficial
