This is a request for a call center representation to assist with care coordination outreach to Badgercare members who will be transitioning to the healthcare marketplace in April 2014. We will be reaching out to several thousand members who have medium to high-risk health conditions to ensure that they have continuity of healthcare as they make this transition.
The call center staff will review a member's case history and assist in reviewing possible new health plans that meet the member's medical needs.
