The world’s trusted source for non-medical services and home care for seniors seeks a multi-talented, professional and caring individual. Position involves managing quality assurance with extensive one on one interaction with our CAREGivers, clients and families. This individual will also institute all aspects of our initial service set-up.

The duties of the Client Care Coordinator:
  • Current license to practice in Massachusetts as an L.P.N.
  • Performing an in-home service call evaluation of any clients requiring services
  • The Client Care Coordinator provides and coordinates individualized care and resources to elders so that they may continue to live in their homes and in the community.
  • Developing a written plan of care in collaboration with the client's and client's family based on the service evaluation
  • Initiating the client's plan of care in accordance with personal care needs specified in the written plan of services
  • Conducting in-home supervisory visits when CAREGiver is present and delivering services to the client
  • Maintaining an ongoing liaison with the professionals and personal care CAREGiver
  • Conduct QA visits with clients in person or over the phone according to the QA frequency guidelines. Conduct follow up as necessary
  • Counsel CAREGivers on quality concerns after visiting clients
  • Conduct client/CAREGiver introductions as scheduled
  • Enter and maintain accurate client and CAREGiver records in BOSS
  • Must be able to remain calm while under pressure and manage stressful situations well
  • Proficient with computers and Microsoft Office
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines.
  • Ability to organize and prioritize daily and monthly work.
  • Must be patient and congenial on the telephone
  • Take initiative to complete duties promptly and assist other office staff as needed
 
Applicant MUST be organized, detail oriented, be able to multi-task, and demonstrate excellent oral and written communication skills and the ability to listen effectively.  Applicant must have a reliable vehicle, driver’s license, car insurance. Complete a criminal background check. And drug test screening.
This is a PART-TIME position.
