  Home Instead Senior Care, a growing home care organization in the Southwest Allegheny County AND the entire Washington County, has a unique opportunity for a Part Time RN or LPN to join our team as our Nurse Coordinator. 

This new role will be dual faceted.  First, you will be leading the Care Transitions Intervention program which facilitates the transition of our clients from hospital/rehab to home.  This unique role will include client coaching visits, CAREGiver team coordination, and data collection/reporting.  Also in this role, you will be supporting our quality of care initiatives by conducting supervisory visits of our high care needs clients.  This will require care plan review, assessment and documentation in addition to CAREGiver coaching.

Position will be Part-Time (approximately 15-20 hrs/week) and will require the ability to conduct evening/weekend QA visits periodically.
Flexibility with the schedule is a must as we will be responding to both planned and unplanned discharges at times.  Training which includes the ability to travel by air will be required in order to obtain the Care Transitions Coach recognition. 

The Nurse Coordinator position will report directly to the Manager of Client Services in our Bethel Park office and will primarily work remotely.  Frequent travel throughout Allegheny County, focusing on Southwest Allegheny County  will be required. Mileage expenses will be reimbursed.

Current PA RN or LPN license and driver's license required.  Previous experience with home & community based services is preferred.  Associates or Bachelor Degree and 2-4 years relevant nursing experience required.
