As the Anticoagulation/Coumadin Case Manager you will receive direction operationally from the RN Supervisor of the Anticoagulation (Coumadin) Clinic. The Case Manager works collaboratively with Anticoagulation (Coumadin) Clinic Team, Director of Cardiology and SMA Primary Care Physicians. Facilitates excellent customer satisfaction while achieving department specific goals. Works directly with patients to manage anticoagulation therapy.
**This is an office based position located in Las Vegas, NV**
**Anticoagulation RN Case Managers must also be willing to do weekend roatation**
Responsiblitlies:
- Responsible for the coordination and quality of care of Anticoagulation (Coumadin) Clinic patients.
- Direct patient care and telephonic management of anticoagulation therapy for patients enrolled in the Anticoagulation (Coumadin) Clinic.
- Evaluate INR responses, identify risk factors, and facilitate brief interruptions in therapy across various disciplines and venues.
- Collaborates with Primary Care Physician to ensure appropriate and safe anticoagulation therapy management.
- Participate in the development and implementation of patient centered care plans.
- Ensure compliance with all SMA/SHS/HPN policies and procedures, state and federal regulations, and standards of practice.
Requirements
Requirements:
- Current, unrestricted RN licensure in the state of Nevada
- Degree in Nursing
- 3 or more years of Clinical RN experience, resulting in excellent clinical skills and judgment
- At least an intermediate level of proficiency with written and verbal communication skills
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment (EMR)
- Intermediate or expert level of proficiency with being able to assess and identify needs and being able to influence and motivate
- Candidate should also be self motivated and flexible.
- Understanding of change and ability to move members along the continuum of change
- Expert conflict management skills
- Able to work under pressure and appropriately prioritize responsibilities
Additional Preferred Assets:
- Bachelor's degree in Nursing preferred
- Experience in Hematology/Cardiology or Home Health preferred
- Would prefer that candidates have prior knowledge in Anticoagulation management.
- Coaching and/or decision support experience
- Strong inbound and outbound telephonic skills, or ability to quickly learn
