We have an excellent opportunity available to join our company as our newest Transplant Case Manager.  In this telephonic position you would counsel prospective organ transplant patients and conduct clinical reviews of members. Responsibilities:


  • Receiving clinical information and making determinations on eligibility to receive an organ transplant

  • Communicating with staff at the transplant facility and assisting members through the transplant process

  • Verifying eligibility information and conducting research in order to educate members on their benefits

  • Conducting clinical reviews of members

  • Reaching out to members to help them navigate information and to explain benefits that are available to them as well as medication, lifetime adherence and compliance to treatment

  • Following up with the member and facility throughout the whole process

  • Maintain the confidentiality of sensitive and protected health information

Requirements

Requirements:


  • An RN with a current, unrestricted license in your state of residence

  • Minimum 3 years of clinical experience within a hospital setting

  • 2 or more years of whole organ and/or stem cell transplant experience  

  • Must live within a commutable distance of a UnitedHealth Group office

  • Computer proficiency, must be able to type and navigate around a Windows environment

  • Strong organizational skills

  • Excellent communication skills, written and verbal

Additional preferred qualifications:


  • Case Management experience

  • Previous experience in a Transplant Coordinator role

  • CCM certification

  • BSN degree
