This United Clinical Services role will be part of our Patient-Centered Care Model team.  The Patient Centered Care Model aligns population health management programs, improves medical/behavioral/social care integration, and shares analytical reports with providers; which improves care management for the consumers with the most complex, chronic conditions who represent the top 5% driving 50% of costs.  This innovative new program leverages marketplace shift from fee for service to shared savings and shared risk arrangements with providers.

As part of an innovation team, this individual will lead the strategic development of our Patient Centered Care Model (PCCM). We are looking for a critical thinking, dynamic, high-energy individual who loves to experiment with new ideas and build business capabilities for those ideas that prove successful:

This role offers an exciting opportunity to lead the development of new and market differentiating population health management capabilities for PCCM. Owning the development function covers the entire gamut from ideation to design to testing to implementation.

1.            Design and develop clinical and analytical capabilities that will enable PCCM, working closely with our Medical Director and clinical resources as well as with key stakeholders in Network (UHN), Healthcare Economics (HCE), or State / Regional Market leaders

2.            Coordinate development of infrastructure around people, process, and technology to achieve scale, working in a matrix environment

3.            Drive execution of PCCM market launch and expansion plan by overseeing timely and successful deployment at new sites and ongoing practice engagement, working closely with a clinical front office as well as a middle office support function

4.            Oversee development of PCCM technology platform, partnering with Technology groups, that enables data analytics and reporting capabilities, which are crucial to successful deployment of PCCM

5.            Lead a team of cross-functional members with operations and business analysis capabilities

Requirements

•             MBA or a Master's degree in Public Health Administration, Public Policy, Economics, or related fields

•             8 to 10 years of total work experience which includes experience in cross-functional business areas such as general management, strategic planning, business development, and people management required

•             Candidates with top-tier consulting firm experience preferred, even with 1 to 2 fewer number of years of total experience

•             Start-up or small business experience preferred

•             Prior experience in an innovation field preferred

•             Health care industry or other service industry experience preferred

•             Critical thinker-this position will need to evaluate merits of new and innovative ideas and make sound decisions

•             Strong influencer- this position will partner with Clinical resources to enable practice transformation

•             Strong relationships-this role will work with Clinical Consulting Directors, Medical Directors, Sr. Leadership as well as business and technology partners to plan, direct and develop clinical programs

•             Strong Communicator-this role will need to be extremely comfortable presenting program objectives, scope, and process across segments

•             Works effectively in an ambiguous environment

•             High degree of emotional intelligence
