OptumHealth is part of UnitedHealthcare. OptumHealth Practitioners conduct assessments in our nursing homes.  We are looking for a Per Diem Nurse Practitioner to complete regulatory and acute care visits in nursing homes approximately 18 hours a week.

Key Responsibilities:


 


Perform acute and regulatory visits for long term care nursing home residents.
Experience working with chronically ill, long term care Medicare patients preferred. Experience with advance care planning.
Providing preventive and primary care for patients in assigned long term care facilities.
Working with primary care physicians to provide the best care possible.
Collaborating with the nursing staff and the patients' families.
Working with the facility's utilization review process.
This position requires ACHA level II background checks (fingerprinting) by the state of FL for all clinicians that have face to face contact with members and will require renewal every five years.


 

Requirements

NP or PA (license and national certification, or the ability to obtain national certification and/or NP license in state of assignment)
Minimum of 1 year of clinical experience in their highest level of education, clinical setting preferred.
Prefer experience in acute care, long term care, and /or palliative care experience working with complex, geriatric residents in long term care facilities.                               
Ability to obtain reliable transportation in order to complete nursing home visit assessments
Excellent administrative, organizational and verbal skills are required.
Professional image and behavior are required.
Strong communication skills with the geriatric or Medicare population.
Ability to navigate on the internet and work with personal computer.
Ability to work independently.
Detail oriented.
Dependable and reliable.
Intermediate level of proficiency in PC-based word processing including basic Microsoft Word and Outlook skills
