About this job
Employees in this function are responsible for the hands-on delivery of care for
patients in their homes. This requires certification as a Nursing
Assistant or Medical Assistant.
Primary Responsibilities:
- Hospice Aide responsibilities and tasks are according to the Plan of Care and include, but are not limited to:
- Providing care consistent with written instructions;
- Assisting with personal hygiene;
- Assisting with ambulation and exercise;
- Reporting changes in the patient's condition and needs to the RN Case Manager;
- Providing nutritional support by assisting in oral feeding;
- Performing other related supportive tasks as specified by organizational policy.
- Communicating with the Hospice RN Case Manager regarding services
provided and significant findings regarding the patient's functional
status and change in care needs. - Documenting the patient's response to care in compliance to the
Evercare Hospice and Palliative Care Policy and Procedures at the time
services are provided. - Submitting documented visit records and completing all job duties in a
manner that meets the accepted standards of practice and the Evercare
Hospice & Palliative Care Policies and Procedures. - Participating with the on-call rotations as assigned.
- Attending and participating regularly in Hospice Staff meetings.
- Contributing to achieving the company's mission.
- Working collaboratively with others to achieve goals.
- Flexible hours.
- Moderate work experience within own function.
- Some work is completed without established procedures.
- Basic tasks are completed without review by others.
- Supervision/guidance is required for higher level tasks.
Requirements
Required Qualifications:
- High School Diploma/GED.
- CPR Certification.
- CNA Certification.
- Ability to travel 100% of the time within Denver Metro area.
- Reliable transportation.
- Valid Driver's License.
- Basic (or higher) level of Computer skills.
Preferred Qualifications:
- 1+ year of Hospice experience.
- Bilingual fluency with English and Spanish.
