Position Description:

We are seeking a Case Manager LPN in the Lehigh Valley, PA and surrounding area. In
this position, you will be completing Patient assessments and Chart
reviews at Nursing homes. After completion of the assessment, you will
report the information back to a Nurse Practitioner. EMR experience is
required. This is a Telecommuting opportunity, with an 8:00 am - 5:00
pm, Monday through Friday schedule. 

Employees in this function
require various Nurse licensure and certification based on role and
grade level. Licensure includes RN and LPN/LVN, depending on grade
level, with current unrestricted licensure in applicable State. These
roles identify, coordinate, and/or provide appropriate levels of care
under the direct supervision of an RN or MD. Function is responsible for
Clinical Operations and Medical Management activities across the
continuum of care (assessing, planning, implementing, coordinating,
monitoring, and evaluating). This includes Case Management, Coordination
of Care, and Medical Management Consulting. Function may also be
responsible for providing Health education, Coaching, and Treatment
Decision support for Members.  

Primary Responsibilities:
  • Ability to work well as a team player.
  • Strong Organizational skills.
  • Effective communication.
  • Basic, structured, standard approach to work.

Requirements

Required Qualifications:

  • High School Diploma/GED.
  • Current, unrestricted LPN License in the State of Pennsylvania.
  • Availability for up to approximately 80% of Travel to Nursing Homes in order to assess Member Care needs.
  • 4+ years of LPN experience (Nursing Home or Short-Term Rehabilitation experience preferred).
  • EMR experience
  • Computer skills, including familiarity with Microsoft Office and ability to navigate within a Windows environment.
  • Reliable transportation, a valid Driver's License, and Automobile Insurance.

Preferred Qualifications:


  • Home Health experience.
  • Case Management experience.
