We have an excellent opportunity available to join our company as our newest Telephonic Transplant Case Manager in Phoenix, AZ. In this telephonic position you would counsel prospective organ transplant patients and conduct clinical reviews of members. Primary Responsibilities:



  • Receive clinical information and make determinations on eligibility to receive an organ transplant 

  • Communicate with staff at the transplant facility and assist members through the transplant process 

  • Verify eligibility information and conduct research in order to educate members on their benefits 

  • Conduct clinical reviews of members 

  • Reach out to members to help them navigate information and to explain benefits that are available to them as well as medication, lifetime adherence and compliance to treatment 

  • Follow up with the member and facility throughout the whole process 

  • Maintain the confidentiality of sensitive and protected health information

Requirements

Required Qualifications:


  • Permanent residence in Phoenix, AZ area with an active, unrestricted RN license in the state of AZ

  • •Willingness to obtain RN licensure in other states, if needed

  • 3+ years of experience in a hospital, acute care, or direct care setting 

  • 1+ years of transplant experience (clinically or as transplant coordinator) 

  • Proficient level of experience with Microsoft Office applications including Word, Excel, and Outlook, and the ability to navigate a Windows environment

  • Strong organizational skills and multitasking abilities 

  • Ability to work independently in a high volume environment

Additional Preferred Assets:


  • Bachelor's Degree in Nursing 

  • Bilingual skills (fluent in Spanish/English) 

  • Case Management experience 

  • Certified Case Manager (CCM) 

  • Experience in utilization and/or concurrent review
