We are looking to a hire a per diem RN  that lives in the downtown LA/Hollywood area. Highly preferred to speak bilingual English and either Spanish, Russian, or Farsi.

Optum is providing health risk appraisals, health assessments, care management and care coordination services to health plans in California who are participating in the Cal MediConnect, Coordinated Care Initiative, Medi-Cal and/or Medicare Advantage programs.  Optum is a premier clinical care management organization, and collaborates with health plans and community agencies to create new LTSS programs. Optum clinicians also operate the Care Plus  model of care for people living in their own homes, community-based settings  and nursing homes.


 


 Position Overview

The Field Care Coordinator has an emphasis in assessment of health risks , functional needs , social needs and behavioral needs among dual eligible populations who may require  long term care services and supports (LTSS) members. In this role, the Care Coordinator would visit members in their homes or other community settings perform health risk assessments.  These assessments will serve as the basis for the development and implementation of a plan of care in collaboration with the member to help them stay healthy and independent in their current living situation


 


Primary Responsibilities:

 

- Administer a face to face health risk assessment in order to identify risk factors for members
- Ensure health risk appraisals are conducted and documented within designated timeframe's
- Function as an advocate for members and assisting in facilitating services
- Build relationships with members and their families; assisting them with proper health choice
- Visit members in their homes, assisted living facilities and other community-based settings
- Delivering a holistic approach to coordinated care based on members needCommunicates effectively to provide case load  status updates to supervisor, requester and other business leaders


- Participate in internal continuous improvement efforts by offering ideas and creative solutions to improve policies, processes and toolsWork collaboratively with peers, leaders, member's health plan and key stakeholders


- Escalates issues as appropriate


- Participates in team meetings

Requirements


Required qualifications: 



• Bachelor's degree in Nursing with a minimum 3  year of clinical or case management 
• Active and unrestricted RN Licensure in California
• Previous field based work experience
• Proficient computer skills including the ability to type and talk at the same time and toggle between multiple screens
• Excellent written and verbal communication skills
• MS Office proficiency; experience with electronic charting
• Effective interpersonal, influence, collaboration and listening skills and drives for results individually and in a team
• Excellent time management, organizational, and prioritization skills and ability to balance multiple priorities
• Ability to work in a self-directed manner in a fast-paced rapidly changing environment
• Reliable vehicle, current and unrestricted driver's license, and proof of auto insurance



Preferred qualifications include:



• Experience in managed care; familiarity with Medicare or Medicaid rules and regulations
• Case management  experience preferred
• Bilingual (English and Spanish, Farsi, or Russian)


Physical Requirements:



• Monday – Friday, field based position requires an unrestricted driver's license and dependable automobile
• Ability to operate basic office equipment (i.e. phone, headset, fax, computer, printer, scanner)
