To that end, the Clinical Coordinator role is a fantastic role to continue your career path in Social and Human Services with a myriad of career options to move up and across the organization.

PA Mentor is seeking a Clinical Coordinator to assess the ongoing need for therapeutic services at a client’s current level of care, coordinate services provided to clients and ensure the quality of care delivered. This individual may also provide therapeutic services to clients.

Qualifications

Key Responsibilities:

  • Provide inidividual and family therapy.
  • Supports the development of and oversees the implementation of service plans for individuals served.
  • Monitors adherence to treatment plans.
  • Reviews placement disruptions, internal movement of clients and on-call activity.
  • Participates in the review and matching of new clients.
  • Investigates internal incidents as required.
  • Regularly reviews clients records to monitor home visits, maintain quality of documentation and ensure compliance.
  • May supervise staff in one or more triads, including hiring and performance management.
  • Reports to a Program Director or Area Director.

Requirements:

  • Master’s degree in Human Service field required
  • One year experience working with population service
  • Licensure and/or certification as required by state
