Objective:
The PRN LPN position is expected to perform a variety of duties that relate to client care for our Medicaid In Home Services program including General Health Evaluations, Medication Set Up, Monitoring skin conditions and periodic Quality Assurance Visits. He/She will continuously evaluate the plan of care through a series of ongoing communications and visits to ensure high quality care and Medicaid participant satisfaction.
 
Responsibilities:                                                                          
 

• Reflect the core values of Bokker Inc. (d.b.a. an independently owned and operated Home Instead Senior Care franchise).
• Meet with Medicaid participants to conduct General Health Evaluations to begin process for In Home Services
• Work with other team members to coordinate various aspects of a client’s care.
• Create and maintain case records as needed
• Plan and execute a schedule that ensures each participant receives their required visits each week for Med Set Up and other related tasks to help ensure personal care needs at home
• Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers.
• Maintain regular attendance at the office and in the home to execute job responsibilities.
• Participate in all CAREGiver meetings (as needed)
• Perform any and all other functions deemed necessary
