This is a work from home and field based position which must be located in the South Memphis area.

Responsibilities for the Telephonic/Field Based Health Coach include:


  • Assist clients and engage members in creating a health vision and goals for their Health & Wellness program

  • Participate in weekly meetings and monthly in-service trainings

  • Demonstrate the ability to deliver effective telephonic and face to face coaching in a manner that promotes behavior change

  • Appropriately adapt telephonic/face to face coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each client served

  • Demonstrate established customer service behaviors and standards

  • Knowledge of disease prevention (cardiovascular and diabetes) and basic understanding of exercise principles, nutrition and weight management

Requirements

Requirements:


  • Experience required in a health related discipline, such as LPN, dietician, social work, health education, exercise physiology, train. Work in a non-clinical health administrative role may be appropriate with demonstrated history of working directly with members to address health related issues.  

  • High school diploma minimum

  • Intermediate level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)

  • Certification in a nationally recognized coaching program or must complete certification by the end of the first year of employment

  • 1+ year of experience required in direct education and/or coaching with individuals for disease prevention, health promotion and behavior change

  • Insight into human behavior and have the ability to communicate and interact with individuals at varying levels of skill and education preparation

  • Willingness to teach, learn and be open to new and innovative ways of delivering prevention lifestyle intervention services

 

Preferred Assets:

 


  • Bachelor's degree

  • Knowledge of TN Medicaid

  • Bilingual in English and Spanish

  • Telephonic coaching

  • Experience working in a field based or community health program
