Telephonic & Field Based Health Coach - Hamblen County - Full-time / Part-time

• Be able to assist clients and engage members in creating a health vision and goals for their Health & Wellness program


• Participate in weekly meetings and monthly in-service trainings


• Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change


• Appropriately adapt telephonic coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each clientele served


• Demonstrate established customer service behaviors and standards


• Minimum of basic knowledge of disease prevention (cardiovascular and diabetes) and basic understanding of exercise principles, nutrition and weight management

• Experience required in a health related discipline, such as LPN, dietician, social work, health education, exercise physiology, trainer, nurse aid.  Work as a non-clinical health administrative role may be appropriate with demonstrated history of working directly with members to address health related issues.  


• High school diploma minimum


• Intermediate level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)


• Either certified in a nationally recognized coaching program or must complete certification by the end of the first year of employment


• 1+ year of experience required in direct education and/or coaching with individuals for disease prevention, health promotion and behavior change


• Must demonstrate insight into human behavior and have the ability to communicate and interact with individuals at varying levels of skill and education preparation


• Willingness to teach, learn and be open to new and innovative ways of delivering prevention lifestyle intervention services

• Bachelor's degree


• Knowledge of TN Medicaid


• Telephonic coaching


• Prior field based or community based experience
