The Hospice CNA (Certified Nursing Assistant) provides for the personal care of the terminally ill patient and perform related tasks in the patient's home according to the patient's Plan of Care, as prescribed by the patient's Physician. Hospice CNAs function in a supportive relationship to professional disciplines and may function as an extension of these professional services with written instructions, appropriate supervision, and specialized Hospice training.

Hospice Certified Nursing Assistants provide care consistent with written instructions. You will see around 5 to 7 patients per day. Your role will consist of overseeing and participating in day-to-day Clinical care of terminal patients and their families; Also responsible for Clinical care and Medical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating), as it relates to end-of-life, palliative care. You will be responsible for connecting and reporting changes in the patient's condition and needs to the RN Case Manager at the beginning and ending of the day. 

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • CNA Certification in good standing with the State of Texas.
  • Current CPR Certification. 
  • Ability and willingness to travel 100% of the time within the local, defined area.
  • Reliable transportation, current Auto Insurance, and a valid Driver's License.
  • The ability to successfully complete a Hospice Aide Competency Skills Supervisor Observation. 
  • Microsoft Outlook proficiency. 
Preferred
Qualifications: 
  • Previous Hospice Care experience.
  • BIlingual fluency in English and Spanish.
