The Case Manager Licensed Practical Nurse will be assigned 2-3 Providers; You will work with these Providers and make Outbound Maintenance calls to them to help set up a Care Plan. You will be in charge of monitoring patients and contacting them after Discharge from the hospital. You are also responsible for managing Inbound orders from the Providers regarding labs, Medical refills, and establishing Home Health services. In this position, you must work well in a Team environment. Other duties, as assigned.
Requirements
- High School Diploma/GED.
- Current and unrestricted LPN License in the State of Arizona.
- 2+ years of Hospital, Acute Care, and/or Direct Care experience.
- Experience with Microsoft Office and ability to navigate within a Windows environment.
- Electronic Medical Records experience.
- Current and unrestricted LPN License in the State of California.
- Prior Case Management experience.
- Certified Case Manager (CCM).
