We are seeking a knowledgeable, compassionate RN to handle patient visits on a PRN basis for day shift hours with our Hospice patients. As a Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.


 


Please note: This position is on a Per Diem/PRN basis working as need to provide coverage for PTO, vacations and sick leave. Ideally, the PRN RN will help conduct visits during the day, but may also assist with evening and weekend visits if available. This position will require travel throughout our service area to conduct visits at our patient's home, nursing home or other facility.


 


Responsibilities for the Hospice Per Diem RN include (as needed):



  • Conducting routine visits at home or skilled nursing facilities

  • Coordinating, assessing, and implementing patients' care

  • Providing accurate and timely documentation of patient services to reflect plans of care

  • Conducting admissions visits

  • Attending weekly interdisciplinary meetings (as needed)

  • Interacting with other group members such as CNA's, LPN's, social workers and dieticians

  • Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

Requirements

Required Qualifications:



  • RN licensure (current, unrestricted) in the state location of the hospice

  • 3 years of clinical experience

  • Reliable transportation including a valid driver's license and auto insurance

 


Preferred Qualifications:



  • Excellent communication skills

  • Experience in long-term care is a plus

  • Background in home care and/or hospice care

  • Computer proficiency with MS Office products including Word and Outlook

  • Proficiency with electronic medical records (EMR) software
