The Per Diem Hospice CNA is responsible for providing care consistent with written instructions.  You will see around 4 to 5 patients a day. Your role will consist of overseeing and participating in day-to-day Clinical care of Terminal patients and their families. This position is also responsible for Clinical Care and Medical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating), as it relates to end-of-life Palliative care.
 
You will be responsible for connecting and reporting changes in the patients' condition and needs to the RN Case Manager at the beginning and end of the day. The travel amount will vary, depending on the location of the candidate. Average commute is around 50 miles a day, and you will be reimbursed for you mileage. You are required to wear scrubs. In order to be successful in this role you must be upbeat and compassionate.
 

Employees in this function are responsible for the hands-on delivery of care for patients in their homes. This requires a Certified Nursing Assistant or Medical Assistant certification.

Primary Responsibilities:

  • Provide care consistent with written instructions.
  • Assist with personal hygiene.
  • Assist with ambulation and exercise.
  • Report changes in the patients' condition and needs to the RN Case Manager.
  • Provide nutritional support by assisting in oral feeding.
  • Perform other related supportive tasks as specified by organization policy.
  • Communicate with Hospice RN Case Manager regarding services provided and significant findings.
  • Document the patients' response to care in compliance to the Evercare Hospice and Palliative Care Policy and Procedures.
  • Work collaboratively with others to achieve goals
  • Moderate work experience within own function.
  • Some work is completed without established procedures.
  • Basic tasks are completed without review by others.
  • Supervision/guidance is required for higher level tasks.

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • CNA Certification in good standing with the State of Massachusetts.
  • Ability to work Per Diem, either 7:00 am - 3:00 pm, 8:00 am - 4:30 pm, or 10:00 am - 6:30 pm.
  • Ability and willingness to travel 100% within the defined area (Waltham, MA to Worcester, MA and/or approximately 50 miles per day).
  • Reliable transportation, a valid Driver's License, and current Auto Insurance.
  • Ability to successfully complete a Hospice Aide Competency Skills Supervisor Observation.
  • Proficiency with Microsoft Outlook.

Preferred Qualifications:

  • Previous Hospice Care experience
  • Bilingual fluency in English and Spanish.
