Position Description:

We are looking for Per Diem Hospice LPNs in the Haymarket, VA area. In this role, you will be sitting bedside and taking care of patients needs. Typically, it is a 12 hour shift, 2 or 3 or more days per week (we do not guarantee hours). These patients could be in a assisted living center or home. You will be working with families and Dr.'s regarding plan of care. You will be administering medicines, giving injections. You will provide availability to location and then they will call you when needed.    

 

Positions in this function require various Clinical Licensure and/or Certification based on role. May include: RN, LPN/LVN, CNA. Non-RNs work under direct supervision of someone with RN or MD Licensure as part of an inter-disciplinary team. Function is responsible for overseeing and participating in day-to-day Clinical care of terminal patients and their families. Function is also responsible for Clinical care and Medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating) as it relates to End of Life Palliative Care.

Primary Responsibilities:

  • Responsible for providing continuous Nursing care to Hospice patients during periods of crisis. Care is provided in-the-home setting.
  • The ideal candidate will have a great amount of flexibility.

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • Current and unrestricted LPN Licensure in the State of Virginia.
  • Ability to travel within a 50-mile radius from Reston, approximately 75% of the time. 
  • 1+ year of LPN experience.
  • Reliable transportation, a valid Driver's License, and applicable Insurance.
  • Availability for flexible Day Shifts (i.e., between 7:00 am - 7:00 pm), seven days a week, as needed.
Preferred Qualifications:
  • Hospice or Home Care experience.
