As a part of our continued growth, we are searching for a new Care Transition Nurse to join our team...


 


The Care Transition Nurse is responsible for assessing and managing patients in the post-acute phase with the goal of improving outcomes, as well as reducing readmissions and complications. Individual services are coordinated for patients whose needs include ongoing medical care, home health and hospice care, rehabilitation services, and preventative services.


 


Primary Responsibilities:



  • Identifies, coordinates, and evaluates appropriate high-quality services, which may be delivered on an ongoing basis

  • Works collaboratively with the patient, physician, family/significant other and providers of healthcare to ensure member's care needs are addressed

  • Works collaboratively with the inpatient team to ensure all discharge planning activities are carried out

  • Identifies acceptable care options to the patient and family, utilizing the Case Management Society of America (CMSA) standards

  • Coordinates resources to reduce fragmentation of services and facilitate the achievement of realistic treatment goals

  • Provides patients with education and training regarding specific health care needs

  • Monitors care, which is easily accessible with no access barriers as related to the patient's available benefits

  • Monitors medications for compliance and potential polypharmacy issues

  • Facilitates early and intensive treatment intervention in the least restrictive setting

  • Facilitates the scheduling of follow up services as appropriate

  • Provides counseling to members who are non-compliant with their plan of treatment

  • Complies with approved time frames and standards for timeliness of Medical Management decision-making

Requirements

Required Qualifications:



  • Current RN or LVN license in the state of California

  • 2 or more years of Clinical Nursing experience: inpatient, outpatient, SNF, LTAC

  • Excellent written and verbal communication skills

  • Computer proficiency in MS Office: Outlook, Word, Excel, PowerPoint 

Preferred Qualifications:



  • Previous Case Management experience

  • Managed Care/IPA work experience

  • Certified Case Manager – CCM

  • Bachelor's Degree in Nursing
