Key responsibilities include:
* Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine needs
* Provides a complete physical assessment and history of current and previous illness(es)
* Provides professional nursing care by utilizing all elements of nursing process
* Responsible for the planning, coordinating and the delivery of quality interdisciplinary home health care services
* Compliance with company standards and federal and state guidelines will be maintained at all times

We seek the following principal qualifications:
* Registered Nurse in good standing within the state
* Recent Home Health experience a must
* Proficient in Medicare, OASIS and PPS
* Strong coding, organizational, and analytical skills
* Solid ability to interact and build relationships with older adults
