NEW GROWTH in Waco Texas (McLennon County) has opened up this great opportunity to lead a team of field based RN Service Coordinators!


  


**This role will work from their home office (telecommute) a few days a week and from nursing home a few days a week**


  


This RN Management role is responsible for:



  • Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)

  • Case management and coordination of care

  • Takes lead role in setting direction and participating in or developing new programs

  • Sets team direction, resolves problems and provides guidance to members of the team

  • Ensures team meets established performance metrics and performance guarantees

  • Adapts departmental plans and priorities to address business and operational challenges

Requirements

 


Required Qualifications:




  • Current, unrestricted RN license in the State of Texas

  • Minimum of 3+ years of experience as a manager

  • Minimum 3+ years of experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Case Manager for an insurance company

  • Solid computer proficiency with basic level of experience with Microsoft Word and with the ability to navigate a Windows environment

  • Working knowledge of Medicare and Medicaid

  • Strong organizational skills and multitasking abilities

  • Must be able to work independently with minimal supervision

  • Must have a reliable vehicle, a current and unrestricted driver's license and proof of insurance

 


Preferred Qualifications:



  • BSN

  • Nursing home experience a plus

  • A background in managed care

  • Case Management experience

  • Certified Case Manager (CCM)

  • Experience/exposure with discharge planning

  • Experience in utilization review, concurrent review and/or risk management

  • Experience in homehealth or hospice

  • CBA experience (community based alternatives)

  • Experience managing staff in a call center environment is a plus
