The Care Advocate for Wellness Recovery will assist members who recently have been discharged from inpatient services and helping them achieve goals for after care planning. The Care Advocate for Wellness Recovery will interact closely with our nursing staff and other clinical staff as necessary.

The chosen incumbents must be flexible to work an 8-hour shift between the hours of 8AM EDT – 7PM EDT.
 
Primary Responsibilities:
•Focus on outpatient services for psychiatric and chemical dependency patients
•Making patient assessments and determining appropriate levels of care
•Obtaining information from providers on outpatient requests for treatment
•Determining if additional clinical treatment sessions are needed
•Collaborating with clinical partners and outpatient providers
•Administering benefits and reviewing treatment plans
•Coordinating benefits and transitions between various areas of care
•Identifying ways to add value to treatment plans and consulting with facility staff or outpatient care providers on those

Requirements

Basic Requirements:
•Independent Licensed Master's degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling OR a Licensed Ph.D. Psychologist OR a RN with a minimum 2 years' experience in mental health
•LCSW, LMFT, LPC OR RN license in the state of GA that is current and unrestricted
•Must have a minimum of 2 years' of demonstrated experience in a related mental health environment
•MS Office proficiency (Word, Excel, Outlook, Internet)
•Must be able to work in a team focused work environment


Preferred Requirements:
•Managed Care background
•Strong understanding of and experience with treatment planning
•Inpatient, Outpatient, and PHP experience
•Previous experience in a telephonic and/or office based role
•Basic understanding of claims and billing
