About this job

SMA seeks an experienced physician with a strong interest in Geriatric Medicine, to join its team of SNFists and NPs in a new Skilled Nursing Facility program.  A member of the Optum family (a subsidiary of United Health Group), SMA is a large integrated physician group that, along with Optum, provides its members with the continuum of health care services, including , but not limited to, primary care, specialty physicians, hospitalists, home health, palliative care and hospice. 


 


The SNFist's responsibilities include direct patient care plus working in a collaborative relationship with NPs in the management of patients admitted to SMA's  skilled nursing facilities. Most of these patients are short stay, rehabilitating after an acute medical illness or surgical procedure. The SNFist will be part of a team of clinicians responsible for optimizing the quality of clinical care, managing the appropriate utilization of resources and interfacing with the other clinical services offered by SMA and Optum. Opportunity exists to help design and build an innovative program that maximizes recovery and prevents unnecessary re-hospitalization.

Requirements

Required Qualifications:



  • Active, unrestricted medical license

  • Board certification in IM or FP

  • 3+ years of post-residency clinical practice experience

  • Ability to work well in a clinical team environment

Preferred Qualifications:



  • Added qualifications in geriatrics
