Optum is currently staffing for a Care Coordination Manager in Boise, ID. The Care Coordination Manager will be responsible for, but not limited to the following duties:   


 




  • Meets at least weekly or as needed with staff for clinical case and performance supervision


  •  Coordinates outreach, education and monitoring activities with Quality and Network in the community


  • Assigns and monitors Caseloads of Field Care Coordinators


  • Carries small caseload of complex members


  • Reviews the quality and timeliness of staff documentation for appropriateness of treatment planning, medical necessity, level of care, care coordination, peer and family peer support


  • Monitors clinical and administrative performance of the assigned Unit.  Identifys development goals for Unit staff, monitors goals and provides resources/feedback, to assist staff in meeting agreed-upon goals


  • Attends psychiatric supervision meetings and consult with the Clinical Director and Chief Medical Officer on clinical cases and achieving department goals


  •  Timely completion of supervisees' performance evaluations


  • Assists with difficult consumer/provider calls


  • Participates in successful implementation of Quality Improvement Initiatives


  • Assists in preparation of clinical reports when requested by Director. Communicates pertinent information about report findings to staff as requested


  •  Monitors outcomes and create action plans to address problematic patterns while maintaining quality of care in conjunction with Director


  •  Other duties as assigned by the Director, including committee participation, in-service opportunities and participation in external workgroups and meetings

Requirements


Basic Qualifications:




  • Registered psychiatric nurse; licensed Masters level mental health professional (LPC, LMFT, LCSW); Care Coordination Manager or licensed Ph.D. (License must be active and  unrestricted)


  • Minimum five years variable clinical experience in a direct services setting with emphasis on acute psychiatric and chemical dependency


  • Minimum two years demonstrated experience in a leadership capacity 


  •  PC proficiency required preferably in a Windows environment (Microsoft Word, Outlook, and Internet)

Preferred Assets:




  • Strong organizational skills


  • Electronic Medical Records (EMR) and Excel experience


  • Experience with complex systems of care in outpatient mental health


  • Managed care and/or insurance experience
