The Lifeprint assessment nurse practitioner (NP) is responsible for providing in-office and occassional in-home assessments of our adult/geriatric members within the Lifeprint network of Maricopa County independent physicians. Through annual wellness assessments, the NP is instrumental in achieving goals focused on the prevention of hospital readmission and reduction of preventable ER utilization.
Duties and Responsibilities
Promote the well-being of our senior members and assist Independent Primary Care Providers (PCP) in the care of their patients by conducting annual physical exam & wellness assessments within multiple settings that include, clinic, patient home and skilled nursing facilites with strong emphasis on senior conditions.
Document the senior assessment in a Lifeprint assessment form and a plan of care in a stand-alone medical record.
Review medications for polypharmacy compliance and educating patients on medications.
Confirm, disagree or mark resolved chronic conditions from patient-specific lists of historical conditions; confirm plan of care developed for patients seen in various health settings.
Complete a Collaborative review of chronic preventative measures for seniors for quality review.

Requirements
Qualifications
The requirements listed below are representative of the knowledge, skill, and/or ability required.
Arizona NP (license and national certification, or the ability to obtain national certification and/or NP license in state of assignment)
Minimum of 3 years clinical experience in their highest level of education, clinical setting preferred.
Family, Geriatrics, or Adult certification
Experience in gerontology, cardiology, internal medicine, or endocrinology
Proficient in assessment and development of treatment plans
Ability to identify quality gaps including medication review for each patient
Arizona Driver's License
Auto insurance in accordance with State of Arizona law
Ability to spend at least one hour with a patient in their home or primary care clinic, which may be in understaffed/remote areas in the precense of pets, or patients / family members that are tobacco users.
Ability to navigate stairs and a variety of dwelling conditions and configurations.
Ability to sit stand and kneel as needed to perform wellness assessments.
Required Knowledge, Skills, and Abilities:
Excellent administrative and organizational skills required.
Professional image and behavior required.
Strong communication skills with geriatric patients, primary care providers, team members and management staff required
Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician.
Detail oriented.
Dependable and reliable.
