Key responsibilities include:


* Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.

* Carry out social evaluations and plan intervention based on evaluation findings.

* Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties, providing crisis intervention when necessary

* Serve as liaison between patients and families/caregivers and community agencies and provide direct oversight and coordination of care with other clinical service providers

* Maintain comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Brookdale Hospice

* Ensure compliance with all state, federal, and CHAP social service regulatory requirements

* Participate in the development of the individualized plan of care and assist an interdisciplinary team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness

We seek the following principal qualifications:


* Master’s degree in Social Work required

* 1 year experience in healthcare setting required

* Hospice experience preferred

* Knowledge of corporate business management government regulations and CHAP standards
* Computer skills with Microsoft programs as well as specialized software

* Strong communications and public relations skills

* Positive attitude and flexibility are essential
