The RN Field Service Coordinator will manage the care of members residing in Nursing Facilities by completing in-person health assessments and coordinating patient care, focusing on disease management and keeping members healthy and independent.  The RN Field Service Coordinator will provide clinical and medical management services, including case management, health assessments, interventions, and discharge planning.  This candidate may identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD.  Work is generally self-directed and not prescribed; this person will work from home when not completing face-to-face member assessments in the field, so it will be important to function in a less structured work environment.

Primary Responsibilities:



  • Engage members in person (nursing facility) to coordinate services, community resources, and treatment needs

  • Travel in assigned territory as needed to complete member assessments; the rest of the time will be spent working in a home-based office

  • Delivering a holistic approach to coordinated care based on the member's needs

  • Identifying unmet needs or gaps in treatment plans as necessary, conducting ongoing assessments and documenting in an electronic file

  • Create, review and revise care plans and focus on disease management

  • Function as an advocate for members and decrease unnecessary hospitalizations

  • Collaborate with the member's PCP and nursing facility to deliver and coordinate necessary services when a member transitions into a nursing facility and throughout the member's stay

  • Build relationships with members and their families; assist them with proper health choices

  • Participate, as needed, with interdisciplinary teams within nursing facilities

  • Ensure cases are documented in a timely manner

  • Other necessary duties as assigned

Requirements

Required Qualifications:



  • RN (License must be current, and unrestricted in state of Texas)

  • Minimum of 3 years recent experience as a case manager, home care RN, or clinical RN experience in acute/rehab/or long term care setting

  • Reliable transportation to travel in assigned territory 

  • High Speed Internet

  • Computer proficiency (Microsoft Word, Outlook, and Internet)

Preferred Qualifications:



  • Nursing home/facility experience

  • Experience with Medicare or Medicaid

  • Bilingual (Spanish)

  • CCM certification
