Due to our expanding business, UnitedHealthcare has multiple opportunities available for RN Case Managers in McKinley and Cibola Counties, NM. 


 


This is a field based/telecommuting role that requires 80% local travel.

Primary Responsibilities:



  • Engaging members telephonically and in-person to coordinate services, community resources, and treatment needs

  • Delivering a holistic approach to coordinated care based on the enrollee needs

  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file

  • Creating, reviewing and revising Plans of Care and focusing on disease management

  • Functioning as an advocate for members and decreasing unnecessary hospitalizations

  • Collaborating with the member's PCP to deliver and coordinate necessary services

  • Building relationships with members and their families; assisting them with proper health choices

  • Ensuring cases are documented in a timely manner

Requirements

Required Qualifications:



  • Current, unrestricted RN license in the State of New Mexico

  • Minimum of 1 year clinical or case management experience

  • Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living

  • Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)

  • Valid Driver's License, access to reliable transportation and proof of automobile insurance

 


Preferred Qualifications:



  • Medicare/Medicaid experience

  • A background working in geriatric special needs

  • A background in behavioral health

  • Experience with electronic charting

  • Bilingual in English/Spanish is a plus
