This is a field based position in and the candidate must live in Summit, Portage, Stark, or Wayne County in Ohio. The hours are generally 8-5 M-F, based on member's needs.

· Provide telephonic care and field based care management to highly vulnerable Medicaid members.
· Offer members health and disease education, and empower them to actively participate in their care
· Consult with members on their medications, review their care plans, address their home care needs and connect them to community resources
· Collaborate with providers, such as primary care physicians or home care providers to ensure there are no gaps in care
· Use strong communication and organizational skills to educate members, and prevent costly gaps in service
Requirements
Required Qualifications:
· Master's degree behavioral health OR Bachelor's Degree Nursing (BSN) OR Associate's Degree in Nursing with ten years of behavioral health experience OR Doctorate-degreed Psychologist
· An active, unrestricted State of Ohio professional license in one of the following:
A. Licensed Professional Clinical Counselor; or
B. Independent Marriage & Family Therapist; or
C. Licensed Independent Social Worker; or
D. Licensed Independent Chemical Dependency Counselor; or
E. Licensed Psychologist; or
F. Registered Nurse
Minimum of 2 years of experience in a related mental health environment
Minimum 1 year experience working with Medicaid population
Minimum 1 year Community Mental Health experience
Proficient computer skills and good working knowledge of Microsoft Word
Must have reliable transportation, valid driver's license, and insurance
Must live in one of the counties listed above
Additional Preferred Assets
· Inpatient experience
· Dual diagnosis experience with mental health and substance abuse
· Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
· Experience working with either the Medicaid or Medicare population
