Employees in this position are responsible for the hands-on delivery of care for patients in their homes and requires Nursing Assistant certification.

Hospice Certified Nursing Assistants provide care consistent with written instructions. You will see around 4 to 5 patients per day. Your role will consist of overseeing and participating in day-to-day Clinical care of terminal patients and their families; Also responsible for Clinical care and Medical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating), as it relates to end-of-life, palliative care. You will be responsible for connecting and reporting changes in the patient's condition and needs to the RN Case Manager at the beginning and ending of the day. 

The travel amount will vary depending on your location, and you will be reimbursed for mileage. You are required to wear scrubs. In order to be successful in this role, you must be upbeat and compassionate.

Requirements

Required Qualifications:
  • High School Diploma/GED.
  • CNA - Certification must be in good standing with the State of Colorado.
  • Current CPR Certification.
  • 6+ months of CNA experience.
  • Ability and willingness to travel 100% of the time within the local, defined area.
  • Ability to work 8-hour shifts, Tuesday through Sunday, weekends included.
  • Reliable Transportation, current Auto Insurance, and a valid Driver's License.
  • Ability to successfully complete a Hospice Aide Competency Skills Supervisor Observation.
  • Microsoft Outlook proficiency, including the ability to send emails and set appointments.
Preferred Qualifications: 
  • Previous Hospice Care experience.
