As a part of our continued growth, we are searching for a new Hospitalist to work at our affiliated Skilled Nursing Facilities.


 


In this role, you will provide care to nursing home patients whether they are permanent residents or are in intermediate convalescent care. You will facilitate communication with patients and with the family members. Monarch Hospitalist Group is affiliated with a variety of local skilled nursing facilities, including:



  • Capistrano Beach Care Center (Capistrano Beach)

  • Carehouse Healthcare Center (Santa Ana)

  • Freedom Village (Lake Forest)

  • Laguna Hills Health and Rehab (Laguna Hills)

  • Manor Care Fountain Valley (Fountain Valley)

  • Palm Terrace Healthcare & Rehabilitation (Laguna Hills)

  • St. Edna Subacute and Rehabilitation Center (Santa Ana) Fountain Valley

While our Skilled Nursing Facility physicians practice mainly at a few primary locations, the flexibility to work at our other local facilities as needed is necessary. The successful physician candidate will share the group's vision of improving the collaboration, coordination and communication with and between physicians and health care professionals caring for patients. In addition, he or she will also recognize the value of coordinating their patients care with specialists and primary care physicians as needed. The physician must be willing to actively participate in improving clinical outcomes through evidence-based strategies and innovative care approaches. The physician must value teamwork and provide clinical oversight and teaching to colleagues and mid-level providers.

Requirements

Required Qualifications:



  • Board Certified or Board Eligible in Internal Medicine or Family Medicine

  • Active and unrestricted California medical license

  • Current and unrestricted DEA certificate

  • Effective communication skills

  • Outstanding organization skills and ability to multi-task

  • Takes initiative, creative, has problem solving ability, is adaptable, and flexible

  • Ability to work without direct supervision and practice autonomously

  • Ability to consistently present a friendly, welcoming and professional public-facing appearance

  • Ability to work in fast-paced environment

Preferred Qualifications:



  • Fellowship training in Geriatric Medicine or Palliative Medicine

  • Post-residency experience, but new residents will be considered
