As the Care Coordinator you will be responsible for assessing health education needs, review and provide follow up education from biometric screenings, teaching classes and providing individual counseling to improve the health & well being of members and hotel employees.  Assure members and their families have access to accurate and up-to-date health information.


 


Primary Responsibilities:



  • Conducts advanced specialized classes and individual consults for members in areas of chronic disease management and health promotion

  • Works with other health educators, staff, physicians and HEW Management in developing, updating and implementing standardized health education programs through curricula development, revision, and evaluation specially for hotel employees

  • Promotes health education through ongoing participation in worksite meetings and projects, task forces and committees and acts as a resource for hotel employees

  • Completes chart notes and employee updates regarding health status and recommendations to providers

  • Maintains monthly MOPS reports, clinical documentation, and evaluation of health education programs

  • Maintains consistent communication with UHC Manager regarding implantation and evaluation of onsite wellness program

  • Coordinates, market and implement health promotional events for hotel 

Requirements

Required Qualifications:




  • Bachelor's degree in health education, health science or related field.  Bachelor's degree in food nutrition or dietetics with Registration in the American Diabetic Association desirable

  • 3+ years of experience in health promotion programs including teaching, curriculum development, program evaluation, behavior modification and lecturing required

  • Excellent oral and written communication skills required

  • Intermediate computer skills in Word, Excel & PowerPoint required

  • Excellent presentation and team building skills required

  • Demonstrates an advanced knowledge of health behavior and adult learning techniques

 


Preferred Qualifications:



  • Certified Diabetic Educator is a highly preferred or able to obtain CDE within 6 months of hire

  • Experience managing diabetic programs

  • Bilingual communication skills are preferred with Spanish as a second language

  • Masters's degree and C.H.E.S. certification preferred
