We are seeking a Hospice
CNA in Haymarket, VA.  You will be visiting around 20 plus patients a
week.  You will usually see around 4 to 5 patients a day and see them 2 to
3 times a week. You will be visiting them at home, assisted living facilities,
nursing facilities, etc.. You will be assisting patients with personal care,
companionship, etc. Once a week you will be responsible for giving updates on
the patients to the case manager and will be responsible for any request they
have in regards to the patient.



 



Primary
Responsibilities:




  • CNAs work under direct
    supervision of someone with RN or MD licensure as part of an
    interdisciplinary team

  • Participates with Hospice team
    members in the development and implementation of the patient's and
    family's plan of care

  • Provides support for and
    reinforces the team's teaching of the patient's caregivers

  • Communicates with Hospice RN
    case manager regarding services provided and significant findings and
    changes in patient condition

  • Applies knowledge/skills to
    activities that often vary from day to day

  • Works with supervisor to solve
    more complex problems

  • Prioritizes and organizes own
    work to meet agreed upon deadlines

  • Performing physical activities
    that require considerable use of your arms and legs and moving your whole
    body, such as climbing, lifting, balancing, walking, stooping, and
    handling of biohazard materials

  • Must have own transportation
    and insurance

  • Must be able to read and speak
    English

Requirements

Required
Qualifications:




  • High School Diploma/GED.

  • Licensed CNA (non-restricted in
    VA).

  • 1+ year of experience as a CNA.

  • Reliable transportation, a
    valid Driver's License, and Insurance.

  • Must be able to drive 50 mile
    radius to Haymarket, VA.



Preferred
Qualifications:




  • Hospice or Home Care experience.

  • Bilingual fluency with English
    and Spanish, Korean, or Chinese.
