OptumInsight is engaged in an exciting opportunity with ten local FQHCs to provide them with support for an ACO pilot program with the Minnesota Department of Human Services.  A key component of the program is working with clinic leadership to drive practice transformation through process change, collaboration and effective use of technology and data.  The Clinical Practices Improvement Coach will be assigned to a specific clinic(s), utilize key data and apply process improvement experience to successfully reach the program targets. 


 


Primary Responsibilities:




  • Assist assigned clinic(s) in driving a quality agenda that effectively meets contract Quality Measures

  • Use the PDSA process (or similar quality improvement model) to focus on practice transformation.

    • Work directly with care teams to establish/embed best clinical and operational practices

    • Implement high functioning, role-based workflows

    • Foster a culture of continuous improvement and patient-centered care

    • Work collaboratively with other Performance Improvement Advisors to establish quality standards across FUHN


  • Utilize software, reports & other analytic outputs to develop data driven interventions to support program goals, including:

    • Identify gaps in care and closing them

    • Improve access to care

    • Reduce inappropriate ER utilization

    • Reduce avoidable admissions and readmissions

    • Identify high risk patients and improve care coordination using techniques such as pre-visit planning, patient outreach,  and self- management skills

    •  Improve patient satisfaction

    • Improve preventive care utilization and selected condition-based quality measure

  • Serve as a liaison between clinical and IT resources on efforts such as panel management, reporting, registries, and clinical decision support

  • Oversee interventions designed for assigned FQ, ensuring processes are effectively designed & documented, staff is trained & results are monitored for progress in achieving target(s).  (Poor performing interventions will be identified, presented to clinic leadership and adjustments made that will ensure continuous improvement.)

  • Review data and trends with clinic leadership and identify intervention opportunities to support achieving program goals

  • Develop & maintain strong, positive relationships with program stakeholders

  • Collaborate with clinic leadership to provide assistance toward clinic's process of achieving MN Health Care Home certification

  • Collaborate with FUHN improvement  colleagues to understand and identify best practices that would benefit overall program

Requirements

Required Qualifications:




  • 3+ years of clinical experience required, preferably in inpatient or outpatient physician practice

  • 2 years of process improvement experience required; how to improve process and creating workflows (i.e. six sigma)

  • 2 years of managing data experience required; use of data, familiarity with data, able to use data for improvement efforts and analysis

  • Experience in either a consulting role or a supporting role in a healthcare IT, practice, or business office setting preferred using data analysis to drive initiatives and/or process redesign

  • Experience in quality improvement, process improvement or organizational change, preferably in a clinical setting. Employee must be physically able to travel locally in metro area at least 50% of the time

  • Excellent oral & written communication skills; able to present at all levels of an organization, including physicians

  • Planning and facilitating meetings and group work

  • Intermediate proficiency in MS Office

  • Experience with data collection, review, analysis and reporting for quality improvement

 


Preferred Qualifications:



  • Prior experience with Federally Qualified Health Centers preferred

  • Knowledge of LEAN improvement practices is desirable

  • Prior experience with analytics tools desirable

  • Experience in medical case management/discharge planning/clinical project management highly desirable

  • RN, LCSW, MPH
