New Jersey MENTOR is seeking a Therapist to provide psychotherapy to individuals to aid a client or consumer in problems of living with the goal of increasing an individual’s sense of their own well-being. Therapists employ a range of techniques based on experiential relationship building, dialogue, communication and behavior change that is designed to improve the mental health of a client or patient, or to improve group relationships (such as in a family).


Description

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.

  • Conducts psychosocial assessments to document history and identify preliminary issues for treatment by interviewing the child and family (if applicable), and transcribing or dictating relevant information for client records.
  • Provides individual, family and/or group therapies using various treatment modalities.
  • Develops individualized treatment plans in coordination with interdisciplinary team to include but may not be limited to: The Department of Children and Families, Family Courts, Statewide Case Management Systems, Juvenile Justice Systems and educational institutions.
  • Works with members of interdisciplinary team to coordinate other treatment initiatives including school performance, permanency,and reunification planning.
  • Develops and coordinates discharge plan based on treatment, indicating all relevant aftercare needs and plans. Researches community resources that would be beneficial to client aftercare needs.
  • Attends regular treatment team staffing and level of care meetings.
  • Maintains current, legible, and adequate medical record documentation in client’s chart. Assures that all required client documentation is current and pertinent,including any foster parent and/or outside agency documentation pertaining to the client.
  • Conducts regular scheduled face to face meetings with client and caretaker in order to monitor the children’s progress and discuss treatment strategies and services. Attends regular staff meetings, personal and professional development coursework, clinical meeting and one-on-one sessions with direct supervisor to process current status of cases.
  • Performs other related duties and activities as required.
Qualifications
  • Master’s degree in human services field with at least three years of experience preferably working with kids
  • Must have a current LSW, LAC, LCSW, LPC, LMFT licensed as required by state of New Jersey.
  • Must have the willingness to travel with Camden, Gloucester, and Cumberland Counties
  • Other education and experience plus licensure as required by state
