DESCRIPTION/RESPONSIBILITES
Case Management Coordinator. Entering assessments, authorizations, scanning/linking. In office staff, call center staff. CMC person that can critically think and understand the concepts of trying to outreach to people and find members. Work with Family Health Plan and Temporary Assistance for Needy Families population. Maintain caseload of about 30 per month.
QUALIFICATIONS
Bachelors degree in Sociology, Psychology, or Healthcare related field
Plus to have experience working with patients in health care setting or case management experience
