About this job

On site (City of Orlando and Field based Liaison responsible for promotion and utilization of clinical programs with specific focus on increasing engagement in Wellness and Clinical/Disease Management programs.  Nurse must be located in the Orlando, FL area and be able to travel in the territory on a daily basis.  You must have reliable transportation as you will be in the field meeting with members and working within the community.  This is a work from home/field based opportunity.


 


 


Responsibilities below:



  • Responsible for education of members on health and condition management (formal and informal), as well as plan specifics and member resources, in collaboration with OptumHealth


  • Referral of employees to available clinical programs including premium designated physicians

  • Create, manage and implement employer specific programs based on employer needs

  • Knowledge of community resources for referral and community outreach

  • Monitors and analyzes medical management activities; provides support to clinical programs; may perform clinical assessments, biometric screenings and clinical audits as needed

  • Clinical interface with facilities and providers for resolution of issues concerning members, benefit interpretation, program definition and clarification.

 

Requirements


  • Undergraduate degree or equivalent experience required

  • Current and Unrestricted RN License

  • 3+ years RN experience required

  • Ability to drive up to 75% in the Orlando territory

  • Reliable transportation required

  • Good computer skills to include Word, PowerPoint, Excel

Preferred Assets:



  • Bachelor's degree or higher

  • Wellness / Disease Management program promotion, education and development experience

  • Nutrition educator

  • Case Management experience

  • field experience

  • Bilingual in Spanish and English

  • Community Outreach experience

  • CCM
