The Hospice Social Worker is a member if the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.


 


Please note: This is a full time position that requires travel throughout our hospice service area.


 


Primary Responsibilities:



  • Visiting patients to assess care and provide pain/symptom control

  • Managing a case load of between 30-45 patients

  • Attending weekly interdisciplinary team meetings

  • Participating in care conferences and working in interdisciplinary teams

  • Supporting the patients' families with funeral arrangements, psychosocial needs, community resources, etc.

  • Maintaining hospice patients' charts and other documentation

  • Participating in a required rotating on call schedule

 

Requirements

Required Qualifications:



  • Master's Degree-level clinician in Social Work

  • Minimum 2 years of social work experience

  • Strong written and verbal communication skills

  • Strong organizational skills

  • Strong computer proficiency, must be able to type and navigate around a Windows environment

  • Must have reliable transportation including valid driver's license and automobile insurance

Preferred Qualifications:



  • Hospice experience

  • Bilingual (English/Spanish)

  • Licensure in the following is highly preferred: LSW, LCSW, LISW and LICSW

  • Experience working with community resources

  • Home health experience

  • Long term care experience
