Primary Responsibilities:



  • Execute upon pharmacy initiatives and provide feedback on DUR, prior authorization, health outcomes, disease management, and other specialty programs

  • Supports the Manager of MTM (Medication Therapy Management) and Med Reconciliation as a resource to health plans, Health Services, Case Management and Behavioral Health areas regarding targeted members with specific pharmacy concerns

  • Review complex cases and high utilizing members

  • Serve as resource to medical directors in preparation for peer-to-peer discussions

  • Work with Care Mangers to coordinate medication therapy post discharge medication reconciliation/formulary support

  • Supports the health plans Medical and Network Management in communicating with key targeted physician groups relative to pharmacy utilization

  • Work with medical directors to identify opportunities for interventions with providers

  • Identify members with polypharmacy issues and develop pharmaceutical care plan recommendations

  • Consults with customers, patients and/or physicians regarding use of medications and potential drug interactions

  • Conducts Quality Assurance efforts related to work accomplished

 

Requirements


Required Qualifications:



  • Pharm D degree from an accredited college or university

  • Current, unrestricted Pharmacist license in the state of Tennessee

  • 3+ years clinical experience 

  • Proficiency in Microsoft software applications that include, but are not limited to, Microsoft Word, Excel, PowerPoint, and Outlook 

  • Demonstrated ability to assist with focusing activities toward a strategic direction as well as develop tactical plans, drive performance and achieve targets 

  • Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action 

  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others 

  • Must live within a commutable distance of our Brentwood, TN office location

Preferred Qualifications:



  • Prior experience working in Medicaid and/or Medicare health care and insurance industry

  • Managed care experience, including regulatory and compliance requirements 

  • Experience working directly with providers and/or patients in a clinical, institutional, or other type of setting
