The Accountable Care Practice Transformation Consultant works directly with leadership of assigned hospitals and physician practices in each Community, supporting practice transformation from a reactive model of patient care to a proactive Accountable Care, analyzing population data to effectively measure, monitor and manage care of patients and present results and outcomes to executive leadership.


 


The Practice Transformation Consultant is responsible for supporting assigned Practice clinical teams in implementing the UnitedHealthcare Accountable Care Communities/Medicaid ACO program.


 


Primary Responsibilities:





  • Improve access to care – use practice level population data to analyze overall capacity/demand for appointments and visit patterns by clinic & overall and assist the Practice leadership to implement process improvements to reach mutual goals and improve patient access



  • Reduce inappropriate ER use – use hospital ADT and daily ER data to analyze trends and work with all Community stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach



  • Reduce avoidable admissions and readmissions – support practices in the effective use of the UHC Transitions and Accountable Care Population Registry to track and manage care transitions of all discharges and ensure follow up with PCP or Specialist within 7 days, helping practice teams engage and coordinate with UnitedHealthcare clinical and hospital teams.  Measure and monitor success of outreach and develop strategies to simplify processes and ensure optimal care for patients



  • Improve care of high risk patients – complete predictive modeling analysis of practice population risk and assist practices in identifying cohorts of high risk/health home patients for focused improvement initiatives



  • Collaboratively establish with practice clinical leadership measurable goals for each cohort to increase access, address current care opportunities and reduce adverse events.  Analyze and report measurable progress against goals at monthly JOC meetings with practices and hospitals



  • Collaborate with UnitedHealthcare teams including the practice Care Coordinators, quality management teams, hospital clinical teams, behavioral health teams to support integrated PCP driven care for our members with practices and hospitals



  • Support practice in implementing process improvements to assist the practice in becoming a certified Patient Centered Medical Home (PCMH), if desired by practice leaders



  • Assist practices in creating work flows to optimize care delivery, introduce best practices improvements, and evaluate outcomes using rapid cycle improvements PDSA methods.  Meet with practice staff at least weekly, to facilitate progress and address barriers



  • Prepare and present progress against goals monthly at on site JOCs with UnitedHealthcare/Practice/Hospital executive teams

Requirements



Required Qualifications:




  • Strong clinical data analytic skills measuring process and outcomes in healthcare

  • Ability to analyze and monitor results of outcome measures and assist physician practice in creating solutions

  • Experience managing change initiatives for health care teams

  • Experience and familiarity with Medical/Behavioral Practices Operations, Hospital Operations, Social Services agencies

  • Strong skills presenting complex data and outcomes to executive teams

  • Practice management/operations experience; hospital collaboration

  • Medicaid managed care experience

  • Strong process improvement skills and experience

  • Ability to organize and thrive in a fast-paced environment

  • Strong interpersonal skills that enable relationship building

  • Effective oral and written communication skills

  • Strong persuasion skills and demonstrated evidence of innovative thinking and program development to make the health care system work better

  • Ability to work independently with minimal supervisory oversight

  • High level of accountability to the program and our customers

  • Operates and performs well in a growth and innovative environment:  Must be a self-starter, has a collaborative and positive team spirit

  • High level of computer literacy with clinical documentation system experience and strong ability to navigate without assistance in multiple computer applications and systems

  • MS Excel and Access database technical skills

  • Highly proficient with Microsoft Excel, PowerPoint, Access, Word, Visio, and Outlook

  • Workflow planning


Preferred Qualifications:




  • Master's Degree



  • Six Sigma/Lean certification



  • Prefer Spanish / other second language relevant to Community
